Job Summary

We are seeking a highly skilled and dedicated Public Health Nurse to join our team at Madera County, CA. As a Public Health Nurse, you will play a critical role in promoting health and preventing disease in our community.

Key Responsibilities
  • Provide nursing, health assessments, education, case management, and counseling services to clients in assigned programs, units, clinics, or districts.
  • Record data and maintain client case files, ensuring accurate and timely documentation.
  • Collaborate with other healthcare professionals and community organizations to ensure continuity of care and services.
  • Develop and implement treatment programs, providing instruction in disease prevention and promoting proper nursing care services.
  • Conduct home visits, meet with clients, and provide assessments, health education, and counseling services.
  • Perform selected diagnostic tests, administer prescribed treatments and medications, and follow established protocols.
  • Collect information and documentation from clients and/or providers, identifying the need for and making referrals to public assistance programs and services.
  • Monitor community health, tracking infectious and preventable illnesses, identifying risk factors, and evaluating community needs.
Requirements
  • Equivalent to a bachelor's degree from an accredited college or university with major coursework in nursing or a closely related field.
  • Possession and maintenance of a valid license to practice as a Registered Nurse in the State of California.
  • Possession and maintenance of a valid California State Public Health Nursing Certificate.
  • Possession of, or ability to obtain by the time of appointment, an appropriate California driver's license.
