Abode Care Partners is seeking a highly skilled and compassionate Medical Director - Advanced Practice Registered Nurse to join our team. As a key member of our interdisciplinary care team, you will provide high-quality, patient-centered care to seniors and individuals with complex needs in various settings.
Responsibilities:
Conduct comprehensive assessments and develop individualized care plans to meet the unique needs of each patient
Collaborate with caregivers and other healthcare professionals to ensure seamless care transitions and optimal patient outcomes
Order and interpret laboratory and diagnostic testing as needed, and provide referrals to other healthcare practitioners when necessary
Ensure accurate and up-to-date patient medical records, and maintain a comprehensive plan of care
Requirements:
Master's degree in Nursing (MSN) from an accredited program
Five or more years of nursing experience, with a focus on geriatric care and medical management
Demonstrated proficiency in medical instruments and equipment, as well as computer-based data management programs and electronic medical record (EMR) systems
Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
