Job Responsibilities and Qualifications

POSITION SUMMARY: As a Clinical Nurse Coordinator, you will oversee the coordination of medical services and discharge planning for designated patient groups. This role involves reviewing patient medical records for compliance and quality assurance, while fostering collaborative relationships with patients, families, healthcare staff, and community agencies. You will ensure seamless communication of patient information across healthcare entities.

Key Responsibilities:

  • Provide orientation to patients and families regarding the case management process, including treatment plans and insurance benefits.
  • Act as the primary educator for patients and families, encouraging their involvement in care discussions.
  • Implement treatment plans that align with patient preferences and needs, facilitating their active participation in the rehabilitation process.
  • Document evaluations and assessments, communicating any social or financial needs of patients.
  • Promote positive patient interactions that enhance service quality.
Required Qualifications

LICENSURE/CERTIFICATIONS:

  • Valid RN license in Texas or a Compact State.

EDUCATION/EXPERIENCE:

  • Associate's Degree or Bachelor of Science in Nursing (BSN).

PREFERRED QUALIFICATIONS:

A minimum of three to five years of clinical experience in an acute care setting, with a focus on case management. Familiarity with case management principles, utilization review, and discharge planning is preferred. Strong analytical skills and the ability to communicate effectively with diverse populations are essential.
