JOB SUMMARY


The primary objective of this role is to deliver exceptional pharmacy consulting services to residents in long-term care facilities. This position aims to enhance therapeutic interventions, educational initiatives, and adherence to pharmacy regulations, thereby ensuring optimal patient outcomes.


QUALIFICATIONS

  • Bachelor's degree from an accredited School of Pharmacy or PharmD
  • Active licensure in the relevant state(s)
  • Consultant license in the state of practice (if applicable)
  • Preferred: Minimum of one year of consulting experience in a long-term care setting
  • Strong analytical capabilities; adept at identifying issues and implementing effective solutions
  • Excellent written and verbal communication skills
  • Basic computer proficiency; familiarity with Microsoft Excel and Word is advantageous
  • Willingness to work extended hours
  • Ability to travel, including overnight stays
  • Capacity for independent decision-making
  • Understanding and interpretation of financial data
  • Proficient in multitasking and completing assignments with minimal supervision
  • Ability to evaluate information and make sound, professional judgments
  • Capability to perform under pressure and deliver results
  • Effective collaboration with facility management and colleagues at all levels; promotes teamwork and professionalism
  • Dependable attendance is essential
  • Must be at least 21 years of age
  • Proficient in English language skills (reading, writing, speaking)

ESSENTIAL JOB FUNCTIONS

  • Oversee and execute all routine tasks, including but not limited to:


o Conducting Monthly Medication Reviews for each resident in assigned facilities, reporting any irregularities to the appropriate personnel through written documentation.


o Evaluating physician medication orders for appropriateness, potential interactions, and administration routes based on relevant healthcare information.


o Implementing corporate clinical initiatives in collaboration with the Pharmacy and Therapeutics committee.


o Collaborating with the Director of Clinical Services to manage cost containment programs.


o Applying knowledge of pharmacy laws and regulations in monthly consultant reviews.


o Assisting with account management and customer service issues.


o Facilitating the development and maintenance of systems that promote safe and effective medication management.


o Educating healthcare professionals through in-service training on regulatory and clinical information to enhance patient care practices.


o Collaborating with facility staff and pharmacy as part of a healthcare team.


  • Assume ownership and accountability for assigned functions and goal achievement.
  • Strive for compliance with internal controls and regulatory standards.
  • Engage in the development and implementation of improvement plans as required, communicating progress and challenges to management.
  • Provide necessary training to staff to ensure successful job performance and facilitate cross-functional awareness.
