About the Role

The Care Coordinator Home Care is a vital member of our patient care operations team, responsible for providing direct casework services to patients and their families within the Vitas Healthcare program of care.

Key Responsibilities
  • Assist patients and families in navigating community resources and accessing necessary support services.
  • Develop and implement personalized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare professionals to ensure seamless care coordination and communication.
  • Provide education and support to patients and families on available resources and services.
  • Conduct thorough assessments to identify patient needs and develop effective care plans.
  • Monitor patient progress and make adjustments to care plans as needed.
Requirements
  • Bachelor's Degree in Social Work or a related field from an accredited institution.
  • Minimum of two years of experience in a healthcare environment, with a focus on community resources utilization and patient contact.
  • Reliable transportation and adequate insurance coverage for driver and passenger.
  • Extensive knowledge of community resources within the specific community served by Vitas Healthcare.
  • Ability to work with minimal supervision and maintain confidentiality.
  • Proficiency in company software programs and ability to learn new systems quickly.
