About the Role:

Kern Health Systems (KHS) is seeking a highly skilled RN Supervisor to lead our Special Programs team. As a key member of our Population Health Management department, you will be responsible for overseeing the management of KHS members in special programs.

Key Responsibilities:

  • Develop and implement strategies to ensure the coordination of care for members in special programs.
  • Collaborate with KHS staff, providers, and community partners to ensure the delivery of high-quality care.
  • Monitor and analyze reports, metrics, and evaluations to ensure the effectiveness of special programs.
  • Provide clinical support and education to non-clinical departments and external partners.
  • Develop and update policies and procedures to ensure compliance with state and NCQA standards.
  • Manage, coach, and mentor staff to ensure the success of special programs.

Requirements:

  • Bachelor's Degree in Nursing, Health Administration, or related field.
  • Minimum 5 years of clinical nursing experience, with 2 years in an acute hospital setting.
  • Experience in building collaborative relationships and working in health care delivery systems.
  • Proficient in various health information systems and Microsoft Office Suite.
