Population Health Special Programs Supervisor RN

We are seeking a highly skilled and experienced RN Supervisor to join our Population Health team. The successful candidate will be responsible for overseeing the management of KHS members in special PHM programs, ensuring the provision of whole person care to specific identified populations and improving health outcomes.
The ideal candidate will have a strong background in acute chronic care nursing, with a minimum of 5 years of clinical nursing experience and 2 years of experience in a supervisory role. They will also possess excellent interpersonal and communication skills, with the ability to work effectively with individuals at all levels within KHS and external stakeholders.
The RN Supervisor will be responsible for developing and implementing strategies to ensure the effective management of KHS members in special PHM programs, including the provision of network education and clinical support, serving as the provider single point of contact for all program services, and developing policies and procedures to ensure compliance with state and NCQA standards.
The successful candidate will be a self-directed individual with the ability to work independently with minimal supervision, and will possess strong attention to detail and the ability to work accurately and at a reasonable rate of speed.
Key Responsibilities:
- Provide network education and clinical support to KHS members and providers
- Serve as the provider single point of contact for all program services
- Develop and implement policies and procedures to ensure compliance with state and NCQA standards
- Manage and coach staff to ensure effective program management
- Collaborate with KHS staff and external stakeholders to ensure effective care coordination and notification
Requirements:
- Active California Registered Nurse (RN) License
- Bachelor's Degree from an accredited institution in Nursing, Health Administration, or related healthcare field
- Minimum of 5 years of clinical nursing experience and 2 years of experience in a supervisory role
- Excellent interpersonal and communication skills
- Ability to work effectively with individuals at all levels within KHS and external stakeholders
