Role Responsibilities:

In this role, you will:

  • Oversee the Health Navigators in your designated area, assuming comprehensive managerial duties.
  • Build and nurture relationships with clinic administrators, healthcare providers, and support staff to resolve inquiries and concerns.
  • Collaborate with Market and Regional Leaders to recruit, train, and supervise a team of Health Navigators (community health workers).
  • Develop and implement educational programs and professional development workshops for Health Navigators in your area.
  • Establish effective communication channels between Health Navigators and the central office.
  • Monitor and evaluate operational and clinical metrics and key performance indicators.
  • Address operational and clinical challenges, providing necessary support to Health Navigators.
  • Participate in continuous improvement initiatives aimed at enhancing operational performance.
  • Conduct regular visits to primary care partner clinics to engage with Health Navigators and connect with key stakeholders.

  • A bachelor's degree is required.
  • A clinical license (Nursing or Social Work) is preferred.
  • This is a full-time position.
