About the Position:

We are currently seeking a highly skilled and compassionate Hospice RN to join our team as a Weekend Hospice RN. This full-time opportunity offers a unique blend of clinical expertise, compassion, and community involvement.

Job Summary:

This position requires a self-motivated and detail-oriented individual who can provide exceptional patient care while working collaboratively with interdisciplinary teams. The ideal candidate will possess excellent communication skills, a strong understanding of hospice principles, and a commitment to delivering high-quality care.

Responsibilities:

  • Deliver direct patient care according to established standards and protocols.
  • Conduct thorough assessments and develop individualized care plans that address patient needs and preferences.
  • Collaborate with interdisciplinary teams to coordinate care and achieve optimal patient outcomes.
  • Communicate effectively with patients, families, and caregivers to ensure smooth transitions of care.
  • Document patient progress and maintain accurate records.

Qualifications:

  • Bachelor's degree in Nursing (BSN) from an accredited program.
  • Current licensure as a Registered Nurse (RN) in the State of Maine.
  • A minimum of one year of recent experience in home health or hospice care.
  • Excellent communication, interpersonal, and problem-solving skills.

Benefits and Compensation:
