Position Overview

Key Responsibilities:

  • Facilitate service coordination for individuals with Autism and Developmental Disabilities
  • Assist clients in navigating available supports and services
  • Engage collaboratively with clients and team members
  • Craft personalized support plans tailored to individual needs
  • Oversee adherence to plans and monitor the overall well-being of clients
  • Maintain accurate documentation of services provided
  • Offer insights on community resources and services
  • Organize meetings to address and support individual needs

Qualifications:

  • Bachelor's degree in social sciences or a related discipline

Requirements:

Possession of a valid Driver's License, necessary clearances, and successful completion of drug screening

Work Environment:

Extended periods of computer use are expected, and travel may be necessary.
