The Supports Coordination Team is essential in delivering services to individuals with Autism, Intellectual, and/or Developmental disabilities, ensuring they and their families receive comprehensive support throughout their lives.
Position Overview
Key Responsibilities:
- Facilitate service coordination for individuals with Autism and Developmental Disabilities
- Assist clients in navigating available supports and services
- Engage collaboratively with clients and team members
- Craft personalized support plans tailored to individual needs
- Oversee adherence to plans and monitor the overall well-being of clients
- Maintain accurate documentation of services provided
- Offer insights on community resources and services
- Organize meetings to address and support individual needs

Qualifications:
- Bachelor's degree in social sciences or a related discipline
