Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Home Care team as a Home Care Coordinator. As a key member of our interdisciplinary healthcare team, you will play a vital role in ensuring seamless transitions of care for our patients.

Key Responsibilities
  • Evaluate patient eligibility and needs for home care services, collaborating with facility care coordinators, social workers, and physicians to find solutions for challenging patients.
  • Conduct comprehensive clinical assessments and develop personalized home care plans, coordinating all in-home services, including professional, paraprofessional services, durable medical supplies, and equipment.
  • Document all required information in patient records, gathering and analyzing data to complete assigned reports.
Requirements
  • RN licensure in New York State required.
  • Five years of experience, with two years in the home care setting, preferred.
  • Excellent interpersonal, written, and verbal communication skills.
  • Experience working in partnership with interdisciplinary healthcare teams.
  • Demonstrated ability to apply sound, objective clinical judgment for cases with varying complexity.
