Patient Care Specialist

Key Responsibilities:- Assist healthcare professionals in providing patient care, including vital sign monitoring, medication administration, and wound care.
- Conduct routine patient assessments, including vital sign checks and medical history reviews.
- Prepare and maintain patient records, including documentation of treatment plans and progress notes.
- Collaborate with healthcare professionals to develop and implement individualized patient care plans.
- Provide emotional support and education to patients and their families, promoting a positive and supportive environment.
- High school diploma or equivalent required; certification in patient care or a related field preferred.
- Previous experience in a healthcare setting, preferably in a dialysis or medical support role.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to maintain confidentiality and handle sensitive patient information with discretion.
- Flexibility to work varying shifts, including evenings, weekends, and holidays.
