Advanced Practice Nurse

```Responsibilities```
- Perform thorough evaluations of patients, including reviewing medical histories, conducting physical assessments, and ordering diagnostic tests.
- Identify and manage both acute and chronic health issues, injuries, and medical conditions.
- Create and execute individualized care plans in partnership with patients and healthcare colleagues.
- Prescribe and monitor medications, assessing patient responses to treatment protocols.
- Educate patients on health maintenance, disease prevention, and chronic condition management.
- Execute minor medical procedures such as venipuncture and wound management.
- Work collaboratively with multidisciplinary teams to ensure seamless patient care and continuity.
- Utilize electronic health record (EHR) systems for accurate documentation of patient interactions and medical histories.
```Experience Requirements```
- A minimum of [4-6] years of practice as a Nurse Practitioner in a healthcare environment.
- Experience in specialties such as family medicine, internal medicine, orthopedics, or neurosurgery is preferred.
- Familiarity with triage, patient ventilation, and medical coding is advantageous.
Qualifications:
- Current Nurse Practitioner license in Texas.
- Master’s degree in Nursing from an accredited institution.
- Certification as a Nurse Practitioner in the relevant specialty area (e.g., Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner).
- Strong clinical acumen and expertise in primary or specialized care.
- Exceptional communication skills with the ability to connect with patients across all age groups.
- Capacity to work autonomously as well as part of a collaborative healthcare team.
