We are seeking a skilled Hospice Care Specialist to join our team at BrightSpring Health Services. As a Hospice Care Specialist, you will play a vital role in providing high-quality care to patients and their families.
Key Responsibilities- Provide medical care and palliative services to patients with terminal illnesses, in compliance with state Nurse Practice Act and under the supervision of a collaborating physician.
- Conduct thorough history and physical examinations to evaluate patients' medical conditions and health histories.
- Complete hospice face-to-face visit requirements to assess eligibility for hospice care.
- Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.
- Maintain accurate and detailed clinical notes.
- Communicate effectively with physicians and participate in interdisciplinary group meetings to meet the medical and emotional needs of patients and their families.
- Ensure compliance with all state and federal legal and regulatory requirements.
- Graduate from an accredited School of Nursing and accredited Nurse Practitioner program.
- Current DEA license, if applicable to state regulations.
- Current nursing license in the state of practice.
- Current CPR certification.
- Master's degree with a minimum of one year of NP experience.
- One year of hospice or palliative care experience preferred.
- Accreditation from an approved certifying body for advanced practice nursing, as required by the state of practice.
- Certification in a specialist area, such as Hospice and Palliative Nursing (CHPN) or Geriatrics, is preferred.
- Commitment to clinical and documentation excellence.
