Job Title: Nurse Practitioner

We are seeking a highly skilled and dedicated Nurse Practitioner to join our team at Heartland Community Health Clinic. As a key member of our healthcare team, you will provide high-quality patient care and contribute to the delivery of exceptional healthcare services.

Job Summary:

The Nurse Practitioner will work under the guidance of the Site Medical Director and collaborating physician to provide nursing and medical services to individuals, families, and groups. This role will focus on health promotion and disease prevention, as well as the evaluation and management of acute and chronic diseases.

Key Responsibilities:
  • Provide care access to walk-in clinic patients in supporting the UI Comp residency program at the clinic
  • Conduct and interpret diagnostic tests, prescribe non-pharmacologic therapies, and provide relevant patient education
  • Maintain accurate, legible, and confidential records
  • Participate in periodic quality assurance review and peer review program
  • Develop and maintain a positive working relationship with patients, family members, department personnel, and outside agencies
Requirements:
  • 1-2 years' experience as a Nurse Practitioner
  • Current State license as a Registered Nurse and licensed Nurse Practitioner and current DEA license
  • Ability to demonstrate the knowledge and skills necessary to provide patient care appropriate to the age of the patients served
