The primary role of the Medical Social Worker II is to provide comprehensive social work care to patients and their caregivers, addressing the social and emotional aspects of their illness. Working under the general direction of the Social Work Manager/LCSW, this role delivers age-appropriate social work services in accordance with agency policy, state, and federal regulations.
Key Responsibilities:- Provides psychodynamic interventions, crisis intervention, grief/bereavement counseling, problem-solving, stress reduction, and healthy coping strategies in individual, family, or group settings.
- Develops and implements individual treatment plans to assist patients and families in coping with and restoring social, emotional, financial, and environmental factors affected by illness.
- Completes psychosocial assessments, partnering with patients to identify needs and develop treatment plans.
- Discusses options for care, including Kaiser resources and external community/government resources, to assist patients and families in developing short- and long-term care plans.
- Collaborates with other disciplines to assess, plan, and provide services for patients using biopsychosocial information.
- Assists patients in advocating for themselves to receive appropriate services within Kaiser and in the community.
- Maintains documentation, records, and data collections, ensuring completion of required documents in a timely manner.
- Master's in Social Work accredited by the Council of Social Work Education.
- At least 2 years of social work experience, with a preference for experience in a healthcare setting.
- Excellent oral/telephone communication skills and written documentation.
- Ability to work independently as a collaborative, supportive team member.
- Computer literacy and experience with automated clinical information systems.
- LCSW certification.
- At least 1 year of post-MSW experience in a healthcare setting.
