Palliative Care Nurse

Hospice of the Piedmont Inc. is seeking a compassionate and skilled Registered Nurse to join our interdisciplinary team. As a Palliative Care Nurse, you will play a vital role in providing high-quality care to patients and their families.
Key Responsibilities- Conduct thorough assessments of patients and families to identify their unique needs and develop personalized care plans.
- Collaborate with the healthcare team to provide comprehensive care, including pain and symptom management, and emotional support.
- Develop and implement care plans that address the physical, emotional, and spiritual needs of patients and their families.
- Provide education and support to patients and families on end-of-life care and advance care planning.
- Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improving patient outcomes.
- Graduate of an accredited nursing program and licensed as a Registered Nurse in the Commonwealth of Virginia.
- Ability to work effectively in a team environment and communicate with patients, families, and healthcare professionals.
- Basic knowledge of Medicare/Medicaid reimbursement regulations and hospice care standards.
- Emotionally mature and caring individual with excellent interpersonal and communication skills.
- Basic computer skills and ability to maintain accurate records.
