Clinical Healthcare Professional for Palliative Care in Charlottesville, VA

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our Palliative Care team at Sentara Medical Group in Charlottesville, VA.
As a member of our interdisciplinary team, you will provide clinical care and support to patients with complex medical needs, including symptom management and end-of-life care. Your expertise will be invaluable in facilitating family meetings and developing discharge plans that align with patients' goals of care.
To be successful in this role, you will need to possess excellent communication and critical thinking skills, as well as the ability to work collaboratively with other healthcare professionals. A strong foundation in palliative care and a Master's degree in a related field (e.g., nursing, medicine) are also essential.

Responsibilities- Provide palliative care consultations and follow-up visits for hospitalized patients with acute palliative care needs.

- Collaborate with an interdisciplinary team to ensure that palliative care is delivered in a comprehensive and patient-centered manner.
- Master's degree in a related field (e.g., nursing, medicine).
- Current licensure as a Nurse Practitioner or Physician Assistant in the state of Virginia.
- Board certification in a related specialty (e.g., palliative care, gerontology).
- Excellent communication and critical thinking skills.
- Ability to work collaboratively with other healthcare professionals.
