About the Role

You'll conduct in-home assessments, evaluating patients' medical histories, reviewing systems, reconciling medications, and performing comprehensive physical exams. Your expertise will help identify diagnoses, inform care management, and facilitate active medical management.

Key Responsibilities:

  • Conduct thorough in-home assessments, including medical history reviews, system evaluations, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing (as appropriate).
  • Identify diagnoses to inform care management and active medical management.
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment.
  • Communicate assessment findings to inform members' primary care physicians of potential care gaps.
  • Recognize urgent and emergent situations and intervene appropriately.
  • Educate patients on disease processes, medications, and compliance.

Requirements:

  • Active NP license or ability to obtain by start date, with an unencumbered license free of any open/unresolved disciplinary actions.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or the ability to obtain national certification and/or NP license in the state of assignment by start date.
  • Ability to complete the physical requirements of the job, including lifting a 30-pound bag, navigating stairs, and various dwelling conditions.
  • Reliable transportation for home visit assessments.

Preferred Qualifications:

  • May be requested to obtain additional licensure in other geographic areas.
  • 1+ years of clinical experience in their highest level of education, clinical setting.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health care or home visit experience.
  • Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages.
  • Ability to travel throughout the state, beyond your assigned region, up to 5% of the time, with advanced notice and potential for additional compensation incentives.
