Atrium Health Charlotte is seeking a highly skilled Patient Care Coordinator to join their team in Charlotte, North Carolina. This role will play a crucial part in the patient's journey from diagnosis to recovery.

Job Summary

The Patient Care Coordinator will be responsible for coordinating the patient's care to increase efficiency and quality of care. This involves providing language-specific education, psychosocial support, and necessary resources to newly diagnosed patients and their families. The coordinator will also serve as a liaison between departments to ensure seamless communication and coordination of care.

The ideal candidate will have excellent communication skills and the ability to work effectively with patients, families, and healthcare teams. A Bachelor's degree in Nursing or a related field is required, with a current license to practice as a Registered Nurse in the state of North Carolina. Certification in a specialized field within two years of being in the position is also preferred.

This is an ongoing staff employment type opportunity with ASAP start dates available.

Responsibilities
  • Provide language-specific education and psychosocial support to newly diagnosed patients and their families.
  • Coordinate appropriate referrals for services as needed.
  • Educate and provide information to new and existing patients and families regarding diagnosis, work-up, treatments, and follow-up.
  • Serve as a liaison for patients and families between departments.
  • Ensure and coordinate new patient referrals, scheduling of exams, procedures, and appointments.
Requirements
  • Bachelor's degree in Nursing or a related field.
  • Current licensure to practice as a Registered Nurse in the state of North Carolina.
  • Certification in a specialized field within two years of being in the position (preferred).
  • Excellent communication skills.
Salary and Benefits

The estimated salary for this role is $85,000-$110,000 per year, depending on experience. Medical benefits, vision benefits, dental benefits, life insurance, 401k retirement plan, and health savings account are all included in the comprehensive benefits package offered by Atrium Health Charlotte.
