Key Responsibilities
  • Coordinate new patient admissions and readmissions, ensuring timely and efficient processing
  • Assist with MDS assessments and other routine documentation, maintaining accurate and up-to-date records
  • Complete assessments of patients with acute illnesses, providing high-quality care and support
  • Coordinate physician and family notification, ensuring effective communication and collaboration
  • Assist with order completion, ensuring accurate and timely processing
  • Participate in on-call responsibilities, providing 24/7 support and coverage
Requirements
  • Current licensure in the state of Wisconsin as a Registered Nurse
  • Degree from an accredited school of nursing
  • Ability to work every other weekend and every other holiday
