Job Summary

We are seeking a skilled and compassionate Adult Outpatient Nurse to join our team at AltaPointe Health. As a key member of our multidisciplinary team, you will provide high-quality patient care, education, and support to individuals with complex mental health needs.

Responsibilities
  • Assess patients' physical and mental health needs, develop and implement individualized care plans, and provide education on disease management and medication adherence.
  • Administer medications, monitor patients for potential side effects, and report findings to the physician.
  • Provide direct patient care, evaluate outcomes, and consult with other specialists as needed to ensure optimal patient care.
  • Maintain accurate and up-to-date patient records, including histories, conditions, treatments, responses, and pain management.
  • Identify and address psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families.
  • Communicate effectively with the physician regarding patients' physical and mental conditions.
  • Perform ancillary testing and tasks as ordered by the presiding physician, including PFTs, EKGs, injections, and audiograms.
  • Administer injections or treatments and perform routine laboratory tests.
  • Provide support for medical assistants to ensure conformance with established standards and procedural requirements.
  • Respond to patients' requests for medication refills and interface with the multidisciplinary team to develop and implement the interdisciplinary treatment plan.
  • Monitor laboratory results and report all abnormal findings to the physician.
  • Provide emergency care as needed, according to Accordia Health Policy and Procedures.
  • Demonstrate the ability to recognize crisis situations and deescalate or resolve them.
  • Participate in general and medical staff meetings as required.
  • Educate the community and patients on health issues to increase health literacy.
  • May conduct infection control for staff and patients when indicated.
  • Perform related duties as assigned.
Requirements
  • Current Registered Nurse licensure in the state of Alabama.
  • Basic Life Support (BLS) certification.
  • Knowledge of adult/geriatric and child/adolescent specialty, to be attained within six months of employment.
  • Demonstration of safe medication administration practices, including IM.
  • Demonstration of knowledge of the principles of growth and development over the life span and the skills necessary to provide age-appropriate care to patients served.
  • Maintenance of current knowledge of medications and their correct administration based on age of the patient and their clinical condition.
  • Knowledge of behavior management, crisis intervention, and interventions common to acute psychotic and non-violent crisis intervention practice.
  • Basic knowledge of appropriate educational methods.
  • Understanding of behavioral, psychological, physical health, and social effects of psychoactive drugs, including alcohol and tobacco on patients and their significant others.
  • Knowledge of effects and treatment of physical/sexual/emotional abuse and neglect issues.
