Job Summary

Chestnut Health Systems is seeking a skilled Targeted Care Specialist to join our team. As a key member of our staff, you will play a vital role in facilitating self-directed recovery for our clients.

Key Responsibilities
  • Develop and implement treatment plans that promote recovery and address barriers to care.
  • Collaborate with primary mental health staff to address clients' chronic health conditions and substance use.
  • Promote the recovery model of treatment, including trauma-informed care, hope, respect, empowerment, health/wellness, and spirituality/connectedness.
Requirements
  • High school diploma or equivalent and five years' supervised mental health experience; OR bachelor's degree in counseling, social work, education, psychology or related human service field; OR a bachelor's degree in any other field plus 2 years' supervised clinical experience in a mental health setting; OR certificate of psychiatric rehabilitation from a DHS-approved program plus a high school diploma or equivalent and 2 years' experience in providing mental health services.
  • Certified Recovery Support Specialist (CRSS) in good standing with the State of Illinois or substance use experience is preferred.
  • Valid driver's license, private automobile insurance and be insurable.
  • Basic computer skills including MS Office, Adobe Acrobat, and an electronic medical record (EMR).
