Insight Global is seeking a dedicated Hospice Registered Nurse Case Manager to become an integral part of a supportive work environment with one of our prominent Hospice and Home Health partners across the nation.
The Hospice RN Case Manager will assume comprehensive nursing responsibilities for implementing and overseeing the care plan tailored for each hospice patient. This role involves making necessary adjustments to enhance both productivity and the quality of care provided.
The selected candidate will utilize their personal vehicle for travel, with mileage reimbursement, to visit various patients within the designated service area.
The RN Hospice Case Manager is expected to manage a caseload of approximately 12-15 patients.
Compensation: The targeted hourly wage for this position ranges from $43 to $47, accompanied by a $2,500 sign-on bonus and additional pay-per-visit incentives.
