Job Title: Hospice RN Case Manager

Insight Global is seeking a skilled Hospice Registered Nurse Case Manager to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality care to patients in their homes.

The ideal candidate will have at least 1 year of clinical experience and a strong understanding of hospice care principles. You will work closely with our clients to develop and implement individualized care plans that meet the unique needs of each patient.

Key Responsibilities:

  • Provide comprehensive nursing care to patients in their homes
  • Develop and implement individualized care plans
  • Collaborate with interdisciplinary teams to ensure seamless care delivery
  • Monitor patient progress and make adjustments to care plans as needed

Requirements:

  • Registered Nurse (RN) licensure in the state of Oregon
  • At least 1 year of clinical experience
  • Active driver's license, dependable vehicle, and insurance

Benefits:

  • Competitive hourly rate: $43 - $47/hour
  • Sign-on bonus: $2,500
