Job Title: Hospice RN Case Manager

Insight Global is seeking a skilled Hospice Registered Nurse Case Manager to join our team and work with one of our largest Hospice and Home Health clients across the US.

The successful candidate will be responsible for executing the care plan for each hospice patient, making necessary changes to increase productivity and quality of care delivered. This role requires the use of a personal vehicle for travel with mileage reimbursement to drive to different homes within Union County, Oregon. The RN Hospice Case Manager will handle a case load of around 12-15 patients.

Key Responsibilities:

  • Develop and implement individualized care plans for hospice patients
  • Conduct regular patient assessments and make necessary adjustments to care plans
  • Collaborate with healthcare teams to ensure high-quality patient care
  • Manage patient records and maintain accurate documentation
  • Communicate effectively with patients, families, and healthcare providers

Requirements:

  • Registered Nurse (RN) licensure in the state of Oregon
  • At least 1 year of clinical experience
  • Active driver's license, dependable vehicle, and insurance
