Nurse Care Coordinator

Job Responsibilities:
Make daily rounds to observe patients and assess the effectiveness of nursing care.
Address issues concerning staff members and take initiative in processing disciplinary action when needed.
Must wear protective equipment as required by department policy and procedure.
Review the Nursing Care Plan reflecting physical, emotional, restorative, rehabilitative, teaching, and goals for the resident.
Provide quality resident care by utilizing the abilities and capabilities of staff members.
Assist and direct Certified Nursing Assistants (CNAs) in organization and completion of workload to meet the needs of the residents, including but not limited to lifting, transferring, repositioning, etc.
Integrate the Nursing Plan with medical care prescribed.
Transcribe and execute physical orders accurately.
Order medication from pharmacy.
Control the counting of narcotics, barbiturates, and counter medications and record properly.
Count syringes accurately and maintain records.
Order supplies for the nursing unit.
Maintain accurate and proper documentation of clinical records.
Evaluate resident care plans for appropriate resident goals, problems, approaches, and revision based on nursing needs.
Schedule time to evaluate performance of all nursing personnel on the unit.
Deliver direct resident care when necessary.
Compensation: We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience.
Requirements:
Licensed Practical Nurse (LPN) license in good standing
Strong communication and interpersonal skills
Able to work effectively in a fast-paced environment
Able to lift, transfer, and position residents safely
Able to maintain confidentiality and handle sensitive information appropriately
