Job OverviewThe successful candidate will be responsible for providing high-quality patient care, promoting independence, and enhancing the overall quality of life for individuals in need of post-acute rehabilitation solutions.
Estimated Annual Salary: $65,000 - $85,000
Responsibilities- Conduct thorough assessments and develop personalized treatment plans tailored to each patient's unique needs and goals.
- Implement evidence-based interventions to improve mobility, strength, balance, and coordination.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and optimal patient outcomes.
- Evaluate progress and adjust treatment plans as necessary to maximize patient satisfaction and success.
- Document all patient interactions, treatments, and outcomes in a clear and concise manner.
- Demonstrated expertise in physical therapy with a strong commitment to delivering exceptional patient care.
- Current licensure to practice physical therapy in the state of employment.
- Graduate degree or greater in physical therapy from an accredited institution.
- Excellent communication and interpersonal skills with the ability to work effectively with diverse populations.
- Able to maintain accurate records and adhere to strict confidentiality guidelines.
