High Fidelity Wraparound Facilitator

Cumberland River Behavioral Health, Inc. is seeking a highly skilled and compassionate High Fidelity Wraparound Facilitator to join our team. As a key member of our multidisciplinary team, you will work closely with families, youth, and community providers to deliver high-quality, family-centered services.
Key Responsibilities- Conduct comprehensive strengths-based assessments of individuals and families to identify needs and develop personalized service plans.
- Collaborate with team members to develop and implement service plans, ensuring a strengths-based focus and respect for family culture.
- Provide consultation and education to providers on the values and principles of the wraparound process.
- Monitor progress towards goals and ensure timely completion of necessary documentation.
- Facilitate team meetings, home visits, and other activities to support family engagement and service coordination.
- Develop and maintain relationships with community providers and resources to ensure access to necessary services.
- Ensure fiscal accountability and maintain accurate records of services provided.
- Bachelor's degree in a Human Services field.
- One year of experience providing TCM services to children/youth with SED and their families.
- Ability to work effectively with diverse populations and maintain a strengths-based focus.
- Strong interpersonal and communication skills, with ability to work collaboratively with team members and families.
- Ability to analyze complex information and define solutions.
- Valid driver's license and reliable transportation.
This position requires the ability to work in a fast-paced, dynamic environment with frequent travel to community locations. The successful candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
