High Fidelity Wraparound Facilitator

Cumberland River Behavioral Health, Inc. is seeking a highly skilled and dedicated High Fidelity Wraparound Facilitator to join our team. The successful candidate will be responsible for delivering care with the highest possible fidelity to the wraparound process, ensuring that the principles of the process are respected and maintained throughout.
Key Responsibilities:
- Assure that care is delivered in a manner that respects the family culture, supports voice and choice, and maintains a strengths-based focus throughout the wraparound process.
- Offer consultation and education to all providers regarding the values of the wraparound process, monitor progress towards goals, and assure that all necessary data for evaluation are gathered and entered.
- Complete a comprehensive strengths-based assessment of the individual and family, work in full partnership with team members to develop and implement a service plan, identify providers of services or natural supports, facilitate team meetings, make regular home visits, monitor all services, and maintain fiscal accountability.
- Help the youth and family to develop and integrate their natural support system and to manage their own services, supports, and plan.
- Maintain a maximum of 10 client caseload.
- Adhere to program deadlines of documentation and care plan protocol.
- Refrain from assuming any duty that is unrelated to and/or interferes with the responsibilities of the position.
- Respond to emergency situations in a timely fashion.
- Demonstrate the ability to work effectively with other team members, as part of a multidisciplinary team, and independently, when necessary.
- Demonstrate ability to follow directions and carry out tasks.
- Demonstrate knowledge of problems specific to the population.
- Report behaviors or occurrences needing clinical assessment and/or intervention.
- Communicate appropriate clinical information to other service providers.
- Other duties may be assigned when necessary.

Additional Responsibilities:
- Possess a valid Driver's License and maintain a safe driving record if required for job performance.
- Work in outside weather conditions, may be off-site from normal office location.
- Occasionally work in a moderate noise level environment.
- Participate in yearly trainings regarding infection control and safety in the environment, including hazardous waste and blood-borne pathogens.
- Attend training regarding handling physical, verbal threats, acts of violence, or other escalating and potential dangerous situations, including when police need to be summoned.
- Attend training regarding medication and self-administration policies and successfully pass a post-test.
- Be certified in Cardiopulmonary Resuscitation within one month of employment and maintain the certification.
- First Aid Training may be required.
