**Job Summary:**

We are seeking a Registered Nurse for a per diem position to work between our facilities in the region. The ideal candidate will have recent experience performing and entering UAS assessments for adult patients, preferably in an assisted living setting.

**Responsibilities:**

* Assess patients in person during hours acceptable to the home/residents
* Enter UAS assessments for adult patients
* Perform paperwork, potentially done remotely

**Requirements:**

* NYS Registered Nurse licensure
* Recent experience performing and entering UAS assessments for adult patients
* Experience in the Assisted Living setting preferred
* Experience with OASIS assessments preferred but not required

**Preferred Qualifications:**

* Experience doing 4449c, MD order, and interim order paperwork
* Experience in doing UAS assessments to achieve RUG scores as it relates to physical/occupational/speech therapy
