Job Summary:

The Per Diem Nurse will be responsible for:

  • Providing direct patient care in a home health setting
  • Assessing patient needs and developing individualized care plans

  • Supervising Home Health Aides and providing education and training as needed
  • Participating in quality assurance and education programs
Requirements:

To be successful as a Per Diem Nurse, you will need:

  • A current license to practice as a Registered Professional Nurse in the State of New York
  • CPR/BLS Certification
  • Minimum of two years of experience in a clinical background as a Registered Nurse
  • Home care experience in a licensed home care service agency or CHHA settings preferred
