**Job Summary**
We are seeking a skilled and compassionate Pediatric Home Health Nurse to join our team. As a home health Registered Nurse and Licensed Practical Nurse, you will provide quality care to pediatric patients in their homes, promoting and maintaining their health and safety. You will work closely with families to learn, provide, and succeed in creating balance for their medically-complex child with consistency, skill, and passion.
**Key Responsibilities**
- Complete assessments, medication administration, and provide therapies, treatments, and interventions as scheduled in accordance with the plan of care and physician's orders.
- Partner with all members of the interdisciplinary team, including the family, to ensure a constant flow of communication regarding the client's cares, changes in condition, developmental goals, etc.
- Provide support and education to clients and families to help them be successful.
- Build rapport and trust with clients, families, or other caregivers, the office team, and peers to facilitate effective communication and build relationships.
- Complete timely charting through our electronic medical record system.
- Keep up to date with the client's plan of care, changes in conditions, physician's orders, family, or caregiver requests, etc.
- Advocate for the client and family to promote and support growth and development.
**Requirements**
- Active Registered Nurse or Licensed Practical Nurse (LPN) licensure, without restrictions.
- Valid driver's license and proof of auto liability insurance coverage.
- CPR certification required but may be obtained after hire.
- COVID-19 vaccine compliance.
- Ability to lift, carry, push, or pull up to 50 pounds.
