Job Summary:

VNS Health is seeking a skilled Nurse Manager to lead our Care Coordination team. As a key member of our organization, you will be responsible for managing the day-to-day operations of our care coordination services, ensuring that our members receive high-quality, comprehensive care in the comfort of their own homes.

Key Responsibilities:

  • Lead a team of care coordinators to achieve excellence in care coordination, quality, and compliance.
  • Develop and implement process improvement plans to enhance the efficiency and effectiveness of our care coordination services.
  • Collaborate with multiple departments to achieve quality care, compliance, and member outcomes.
  • Use software systems, including Excel dashboards, to manage processes, data, and performance metrics.
  • Coach team members on productivity and performance, fostering collaboration and decision-making.
  • Maintain a varying compliance environment to ensure that all policies and guidelines are adhered to.

Requirements:

  • License and current registration to practice as a Registered Professional Nurse in the state of New York.
  • Bachelor's Degree in nursing or Master's Degree in Social Work.
  • Minimum five years of experience in home care, care management, or a related field.
  • Minimum of two years progressive experience in clinical management.
  • Demonstrated knowledge of NYS regulations regarding Medicare or Medicaid Managed Long Term Care health plans.
