Position Overview:
The role of the Hospital Care Coordinator involves a variety of responsibilities aimed at ensuring optimal patient outcomes and efficient care delivery. Key responsibilities include:

  • Linkage and Advocacy: Facilitate connections and provide advocacy for patients to enhance their functioning and well-being.
  • Resource Management: Utilize a comprehensive understanding of available resources to promote quality and cost-effective healthcare outcomes.
  • Caseload Management: Effectively oversee and manage an assigned caseload, tailored to the unique needs, strengths, and preferences of each individual.
  • Discharge Planning: Coordinate efforts for discharge planning through thorough assessment, referral, and collaborative interdisciplinary planning.
  • Adaptability: Demonstrate the ability to adjust to shifting priorities in a dynamic healthcare environment.

Qualifications:
Certifications:

  • Mandatory: RN (Registered Nurse)
  • Preferred: ACM (Case Management), CCM (Case Manager)
Education:
  • Mandatory: Associate's Degree in Nursing
  • Preferred: Bachelor's Degree in Nursing
Experience:
  • Mandatory: Minimum of 2 years in Acute Care
  • Mandatory: Minimum of 2 years as a Registered Nurse

Facility: BayCare Health System, Care Coordination Department
Location: Winter Haven
Status: PRN, Non-Exempt
Shift Hours: 8:00 AM - 4:30 PM
Shift: Days
Weekend Work: Occasional
On Call: No

Remote Work: This position does not allow for remote work.
