Position Overview

General Summary

With minimal supervision, this role involves delivering a high level of comprehensive health services to a designated patient demographic in partnership with medical personnel. Adheres to established nursing and medical protocols, procedures, and practices, while providing specific care instructions to nursing and support staff.

Key Responsibilities

The following points outline the essential functions of this position. This list is not exhaustive and may include additional duties as assigned.

  1. Conducts medical assessments and evaluations, including diagnosis, treatment, follow-up, consultations, and health education tailored to the patient's age and circumstances.
  2. Collects patient histories and formulates care plans, ensuring thoroughness and accuracy in documentation.
  3. Requests, acquires, and interprets relevant diagnostic tests.
  4. Creates, orders, and documents suitable treatment plans and care strategies, including medication prescriptions based on a detailed review of health history and diagnostic findings.
  5. Communicates care plans effectively with relevant healthcare providers, including collaborating physicians, primary care providers, nursing staff, and case managers.
  6. Engages with patients and their families to explain care plans, disease processes, new treatment strategies, and medication instructions.
  7. Administers medical care to patients with illnesses and injuries in accordance with accepted standards.
  8. Provides health education to patients and families as necessary and appropriate.
  9. Records medical findings and care delivered in the patient's clinical documentation, including laboratory and imaging study results.
  10. Educates patients on health maintenance practices and supplies educational resources as needed.
  11. Collaborates with physicians and other healthcare team members regarding patient health needs, outcomes, and the effectiveness of care plans.
  12. Initiates contact with other healthcare providers as necessary.
  13. Ensures the confidentiality and security of all protected health information, disclosing only what is essential for job functions.
Qualifications

Education and Experience

  1. Master's degree in Nursing with completion of an accredited Nurse Practitioner Program is required.
  2. Current state licensure as a Nurse Practitioner is mandatory.
  3. Current state licensure as a Registered Nurse is required.
  4. Current BLS certification or acquisition prior to engaging in direct patient care is necessary.
  5. Experience is preferred but not mandatory.
  6. Eligibility for DEA and CDS is required.

Knowledge, Skills, and Abilities

  1. Demonstrates knowledge and skills necessary to provide appropriate care for the patient populations served. Exhibits understanding of growth and development principles across the lifespan and possesses the ability to assess data reflective of the patient's needs relative to specific population and age requirements.
  2. Exhibits strong verbal and written communication skills essential for interaction with patients, families, departmental units, and medical and nursing staff on critical matters. Proven effective interpersonal skills are required.
  3. Maintains current hospital safety and other mandatory training.
  4. Ensures immunizations are up to date as recommended for the practice area.

Patient Safety Commitment

Prioritizes patient safety in job functions and actively participates in hospital, department, or unit patient safety initiatives.

  1. Takes corrective action to address identified risks to patient safety.
  2. Reports adverse events and near misses to the appropriate management authority.
  3. Identifies potential risks in processes, procedures, and devices, communicating these to responsible parties.
