Home Health Physical Therapist

We are seeking a highly skilled and compassionate Physical Therapist to join our team at Pinnacle Home Health Therapy Care. As a Physical Therapist in our Home Health Care program, you will have the opportunity to work with adult and geriatric patients in their homes, providing personalized care and rehabilitation services.
Job SummaryThis is a full-time or part-time (24 to 40 hours a week) position that offers a high degree of flexibility. You will schedule directly with your patients, allowing you to determine your own schedule and work at your own pace. Our Physical Therapist position is ideal for those who value independence and autonomy in their work.
Responsibilities- Provide physical therapy services to adult and geriatric patients in their homes
- Develop and implement personalized treatment plans to meet the unique needs of each patient
- Conduct thorough assessments and evaluations to identify areas of improvement and set realistic goals
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
- Maintain accurate and detailed records of patient progress and treatment plans
- Master's or Doctorate degree in Physical Therapy
- Current licensure to practice Physical Therapy in the state
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
- Flexibility to work a variety of schedules, including evenings and weekends
