About the Role

Galileo is a team-based medical practice dedicated to improving the quality and affordability of healthcare for all. As a Behavioral Health Advocate, you will be an integral part of our collaborative care team, working directly with patients who have primary behavioral health disorders and comorbid medical diagnoses. Your primary goal will be to facilitate meaningful patient engagement and person-centered care, working closely with primary care providers and licensed social workers to maintain care plans tailored to patients' needs and behavioral health treatment goals.

Key Responsibilities:

  • Travel to patients' homes to provide longitudinal care and conduct community-based outreach to engage high-acuity patients
  • Meet with patients in various settings, including their homes, shelters, and community agencies, to provide care coordination and support
  • Preliminarily assess patients for acute behavioral and physical health needs, generating referrals to providers and community-based resources as needed
  • Document patient interactions and care coordination activities in a comprehensive manner, ensuring compliance with professional standards and regulatory guidelines

About You

We seek passionate individuals who are invested in delivering quality care to individuals with complex behavioral health disorders. You should be excited about social advocacy and aware of how the stigma of having such a disorder can impact an individual on a systemic, social, and familial level. A background in community and behavioral health, combined with an enthusiasm for applying those tenets in a new and creative way, is highly desirable. Additionally, you should be comfortable working in a multidisciplinary care team environment and have a deep knowledge of social benefits programs and local resources.

We offer a competitive hourly rate of $28-32/hour, based on prior experience, performance, and market dynamics. This is a field-based role, with 60% of the time spent in the field seeing patients and the rest of the time working remotely. You will be required to commute to patient visits through personal transportation or with our team in our mobile unit.
