Job Title: Occupational Therapy AssistantJob Summary:

Discovery At Home is seeking a skilled Occupational Therapy Assistant to join our team in Venice, Florida. As a key member of our home health and outpatient team, you will provide high-quality care to our patients, working under the supervision of our Occupational Therapist.

Responsibilities:
  • Provide client care services, including evaluation and development of the rehabilitative plan of care, under the direction of the Occupational Therapist.
  • Assist in the preparation of clinical and daily progress notes, ensuring timely submission within 72 hours.
  • Participate in the preparation of periodic summary reports and instruct clients and family members in exercise programs and self-care activities.
  • Identify potential issues with medication and report them to the Occupational Therapist.
  • Consult with the Occupational Therapist regarding the outcome of home visits and report physical findings in written form.
  • Assist in the evaluation and development of the rehabilitative plan of care and in periodic re-evaluation as indicated.
  • Participate in staff meetings and case conferences, and instruct other health team personnel, including Home Health Aides, on certain phases of occupational therapy.
  • Write daily progress notes and submit them within 72 hours, and prepare periodic summary reports as indicated.
  • Assume responsibility for self-development through educational programs, attendance at workshops and conferences, and individual research and reading.
  • Participate in Quality Assessment and Performance Improvement activities as assigned.
  • Attend all patient care conferences as scheduled, and complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, visit notes, and significant changes in condition in accordance with agency-defined time frames.
Qualifications:
  • Must be a graduate of an approved school giving a course of not less than two years for Occupational Therapy Assistants.
  • Must be licensed or registered by the State.
  • Must have a criminal background check.
  • Must have CPR certification.
