About the Position

The Cancer Patient Advocate serves as a vital link between patients, families, and healthcare providers, ensuring seamless communication and coordination of care. As a member of our interdisciplinary team, you will work closely with patients, families, and healthcare providers to provide holistic care and support throughout the cancer journey.

Responsibilities

  • Assess and prioritize patient needs, developing individualized care plans to address physical, emotional, and social concerns.
  • Collaborate with healthcare teams to coordinate care, communicate effectively with patients and families, and provide education and resources to promote informed decision-making.
  • Use critical thinking skills to identify and address potential barriers to care, such as financial or social determinants of health.
  • Document patient interactions and outcomes, maintaining accurate and timely records to inform care decisions.

Requirements

  • Degree in Nursing from an accredited institution.
  • Minimum 1 year of experience in oncology nursing or a related field.
  • Current licensure as a Registered Nurse in the state of Texas.
  • Basic Life Support (BLS) certification.
