Job Title: Behavioral Health Specialist

Job Summary:

The Behavioral Health Specialist will provide primary community support and case management to clients of all ages, working collaboratively with our interdisciplinary team to deliver exceptional care. This role requires a strong understanding of community resources, excellent communication skills, and a passion for empowering individuals and families to achieve their full potential.

Key Responsibilities:
  • Conduct intake screenings to assess client needs and develop personalized service plans.
  • Provide case management services, including referrals to community resources and advocacy to support client goals.
  • Maintain accurate records and participate in regular team case reviews to ensure seamless care coordination.
  • Collaborate with clients, families, and community providers to promote a positive and supportive environment.
  • Stay up-to-date with relevant laws, regulations, and best practices in behavioral health to ensure high-quality care.
Requirements:
  • Associate's degree in Behavioral Health, Health Sciences, or a related field.
  • Two years of experience in a related field, with a strong focus on community support and case management.
  • Eligibility for Agency Affiliated Counselor Registration from the State Department of Health.
  • Bilingual in English/Spanish is strongly preferred.
