About the Role

We are seeking a skilled Speech Language Pathologist to join our Home Health team. As a Speech Language Pathologist, you will work with patients in their homes to evaluate, direct, and provide speech/language pathology services. Your expertise will help patients recover from injuries, illnesses, or surgeries, and regain their independence.

Key Responsibilities
  • Evaluate patients' speech and language abilities to identify areas for improvement
  • Develop and implement individualized treatment plans to address patients' needs
  • Provide speech and language therapy to patients in their homes
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document patient progress and maintain accurate records
Requirements
  • Master's degree in Speech Language Pathology or related field
  • Current licensure and certification in Speech Language Pathology
  • Minimum of six months experience as a Speech Language Pathologist
  • Home Health experience preferred
  • Excellent communication and interpersonal skills
