Job Summary

We are seeking a skilled Occupational Therapist to join our Home Health team. As an Occupational Therapist, you will play a vital role in helping patients regain independence and improve their quality of life.

Key Responsibilities
  • Conduct comprehensive evaluations to assess patients' physical, social, and mental functions.
  • Develop and implement individualized treatment plans to address patients' needs and goals.
  • Provide therapy interventions using evidence-based techniques and protocols.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements
  • Bachelor's Degree in Occupational Therapy or equivalent required.
  • Master's Degree in Occupational Therapy or related field preferred.
  • Current Occupational Therapy licensure in the state of practice required.
  • Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification required.
