Job Summary

We are seeking a skilled Occupational Therapist to join our Home Health team. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and achieving exceptional outcomes.

Key Responsibilities
  • Conduct comprehensive evaluations to identify patient needs and develop individualized treatment plans.
  • Provide evidence-based occupational therapy interventions to promote patient independence and functional ability.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and effective communication.
  • Supervise and mentor occupational therapy assistants to ensure high-quality patient care.
Requirements
  • Bachelor's degree in Occupational Therapy or equivalent required; Master's degree preferred.
  • Current Occupational Therapy licensure in the state of practice required.
  • Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification required.
