Overview:
At Madison Grove Post Acute, we are looking for a dedicated and empathetic Rehabilitation Specialist to enhance our team. In this role, you will assist patients in recovering mobility and alleviating discomfort following injuries or medical conditions.

Key Responsibilities:
  • Assess patients' medical backgrounds and conduct thorough physical evaluations.
  • Create tailored treatment strategies that align with each patient's individual objectives.
  • Administer hands-on rehabilitation techniques, including exercises, stretches, and manual therapies.
  • Instruct patients and their families on effective at-home care practices.
  • Track patient progress and modify treatment plans as needed.
  • Work collaboratively with other healthcare providers to ensure comprehensive patient care.
  • Keep precise and current patient documentation.

Qualifications:
  • Current state licensure or certification as a Rehabilitation Specialist.
  • Preferred experience in pediatric care, hospital environments, or acute care settings.
  • Knowledge of trigger point therapy, medical terminology, physiology, and anatomy.
  • Strong communication skills to engage effectively with patients and their families.
  • Ability to work autonomously as well as within a multidisciplinary team.
  • Excellent problem-solving and analytical skills.
