Job Description:
As a Palliative Nursing Professional with Tandym Group, you will play a vital role in enhancing the quality of life for patients and their families. This position requires a strong background in hospice care and the ability to work collaboratively as part of a multidisciplinary team.

About the Role:
In this capacity, you will be responsible for providing palliative care services to patients in a community setting in Brooklyn. Your key responsibilities will include conducting assessments, developing care plans, and making recommendations based on patients' needs.

Responsibilities:
  • Assess patients' physical and emotional needs, identifying areas for improvement and developing targeted care plans.
  • Ensure accurate and timely electronic documentation of patient interactions, maintaining clear records of care delivered.
  • Develop and implement comprehensive care plans that cater to individual patients' needs and preferences.
  • Making informed recommendations for care based on patients' needs, ensuring optimal patient outcomes.
  • Collaborate with patients' Physicians to facilitate plan of care changes and ensure seamless delivery of care.
  • Train and supervise Home Health Aides and homemakers, promoting excellence in care delivery and patient satisfaction.
  • Perform other duties as needed to support the team and achieve organizational goals.

Requirements:
  • Demonstrated expertise in hospice or hospital settings, with a minimum of 1 year of relevant experience.
  • NYS Registered Nurse (RN) license, ensuring compliance with regulatory requirements.
  • Bilingual in Cantonese or Mandarin, enabling effective communication with diverse patient populations.
  • Strong background in hospice and/or home care, with a passion for delivering compassionate patient-centered care.

Salary:
The estimated salary range for this full-time opportunity is $90,000 - $115,000 per annum, commensurate with experience. Benefits include comprehensive health insurance, paid time off, and opportunities for professional growth and development.
