Job Title: Case Manager II

Job Summary:

Heartland Family Service is seeking a skilled Case Manager II to join our team. As a Case Manager II, you will provide direct care and support to residents in a residential substance abuse treatment facility, serving women and their children. This is a dynamic and challenging role that requires a strong commitment to trauma-informed care and a passion for working with individuals and families affected by substance abuse.

Key Responsibilities:

  • Provide supervision and support to residents as part of the 24-hour direct care staff
  • Assist staff in implementing case management activities and goals with and for clients
  • Participate in interdisciplinary family/case staffing to obtain information, provide information, and assist with service planning as needed
  • Complete intake information with incoming clients
  • Review case management activities, goals, and rules with Program Coordinator
  • Document client activity, progress, and needs per established record-keeping procedures
  • Implement case management plans and provide support services for program participants under the direction of Lead Case Manager and Program Coordinator
  • Follow up with individual clients and update goal planning as client goals change
  • Responsible for maintenance and upkeep of facility, including monitoring residents' activities
  • Orient clients to WCC rules and address client non-compliance as necessary to create a safe and secure environment for other residents
  • Work as a team member with other program staff to provide trauma-informed care to clients
  • Assist in facilitating educational participant support groups and children groups as assigned
  • Network with community providers and other agency programs in order to assist clients in accessing needed community resources
  • Evaluate efficiency and effectiveness of community service providers on an ongoing basis to ensure clients are receiving quality care
  • Develop supportive relationships with clients
  • Advocate for clients in the community
  • Able to work independently with little supervision
  • Maintain required records and statistical information
  • Display a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency
  • Cooperate and collaborate with program area staff, volunteers, and other Agency staff
  • Be dependable and punctual regarding scheduling and attendance
  • Abide by all specific program and Agency procedures, policies, and requirements
  • Develop personal and program-related skills through participation in internal and external training opportunities
  • Create, maintain, and share as appropriate a dynamic self-care plan
  • Strive to make connections between the agency and the larger community whenever possible in order to contribute to the agency's ongoing fundraising and friend-raising efforts

Requirements:

  • Bachelor's Degree in Human Services, Psychology, Social Work, Sociology, or a related field preferred
  • Two or more years' experience in the human services field
  • Alcohol/drug abuse experience preferred
  • Valid driver's license/acceptable driving record
  • Bilingual-Spanish speaking abilities preferred
