Case Manager II

Optum, a leading healthcare company, is seeking a skilled Case Manager II to join our team in Omaha, NE. As a Case Manager II, you will play a vital role in providing home-based services, education, and support to families, including household management and emotional/behavioral regulation.
Key Responsibilities:- Provide home-based services, education, and support to families
- Travel independently for home visits, school, office visits, or other necessary facilities
- Participate in monthly observations and weekly supervision with MHP Supervisor
- Schedule psychiatric and PCP appointments to maintain compliance with standards
- Coordinate services within and outside the clinic (counseling, medication management, welfare, vocational services)
- Implement and follow intervention plans written by a therapist
- Submit documentation within 24 hours of service, follow mandatory reporting laws, and maintain weekly billing
- Current CPR & First-Aid Certification or ability to complete BLS certification within 30 days of hire
- 1+ years of experience in the Human Services field
- Experience working with computers for professional communication and medical documentation - Excel, Outlook, Athena RMS (or other EHRs)
- Access to reliable transportation, as well as a valid driver's license, that will enable you to travel to client and/or patient sites within a designated area
- Experience with clients that present with various issues; substance abuse, domestic violence, trauma recovery, anxiety, depression, relationship problems with family or significant others, stress management, homelessness, and/or lack of resources
- Ability to define problems, collect data, establish facts, and draw valid conclusions
- Ability to interpret technical instructions in mathematical or diagram form and deal with abstract and concrete variables
- Excellent written and oral communication skills
