With A Little Help is seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will be responsible for overseeing and coordinating the care plans and services provided to our clients.
Key Responsibilities:
- Coordinate and monitor the delivery of care services to clients
- Assess client needs and develop individual care plans
- Ensure that appropriate caregivers are matched with clients
- Schedule and coordinate caregiver visits and shifts
- Provide support and guidance to caregivers
- Maintain accurate and updated client records
- Collaborate with healthcare professionals and community resources
- Ensure compliance with regulatory guidelines and company policies
Secondary Responsibilities:
- Assist in the recruitment and onboarding of caregivers
- Support compliance and other general office duties
- Provide fill-in caregiver support as needed
Requirements:
- 1-3+ years of home care scheduling experience
- Proven strong organizational skills with the ability to multitask effectively
- Experience building workflows for efficient tracking, engagement, and communications with caregivers
- Excellent communication and interpersonal abilities
- Strong attention to detail and the demonstrated ability to work independently, set priorities, manage time and multi-task
- Expert experience with Microsoft Office programs, especially Outlook, and a solid understanding of computers
- Proof of eligibility to work in the United States and ability to pass a criminal background check
Preferred Qualifications:
- Previous caregiving (HCA/CNA) experience is a huge plus (1+ years)
