Job Description

With A Little Help is seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will be responsible for overseeing and coordinating the care plans and services provided to our clients.

Key Responsibilities:

  • Coordinate and monitor the delivery of care services to clients
  • Assess client needs and develop individual care plans
  • Ensure that appropriate caregivers are matched with clients
  • Schedule and coordinate caregiver visits and shifts
  • Provide support and guidance to caregivers
  • Maintain accurate and updated client records
  • Collaborate with healthcare professionals and community resources
  • Ensure compliance with regulatory guidelines and company policies

Secondary Responsibilities:

  • Assist in the recruitment and onboarding of caregivers
  • Support compliance and other general office duties
  • Provide fill-in caregiver support as needed

Requirements:

  • 1-3+ years of home care scheduling experience
  • Proven strong organizational skills with the ability to multitask effectively
  • Experience building workflows for efficient tracking, engagement, and communications with caregivers
  • Excellent communication and interpersonal abilities
  • Strong attention to detail and the demonstrated ability to work independently, set priorities, manage time and multi-task
  • Expert experience with Microsoft Office programs, especially Outlook, and a solid understanding of computers
  • Proof of eligibility to work in the United States and ability to pass a criminal background check

Preferred Qualifications:

  • Previous caregiving (HCA/CNA) experience is a huge plus (1+ years)
