Key Responsibilities

As a Case Manager II, your key responsibilities will include:

  • Coordinating patient care from admission to post-discharge
  • Overseeing interdisciplinary plan-of-care decisions
  • Managing resources and coordinating patient care
  • Facilitating team conferences and coordinating treatment plan modifications
  • Completing case management addendums and required documentation
  • Maintaining knowledge of regulations, company policies, and department operations
Qualifications

To be successful as a Case Manager II, you'll need:

  • A minimum of an Associate Degree in Nursing (RN licensure with BSN preferred) or a bachelor's degree in a related field
  • 2 years of rehabilitation experience preferred
  • CCM or ACMTM certification required or must be obtained within 2 years of being placed in the Case Manager II position
